Since April 1, 2025, the facilities providing elderly care in NYC have shifted how they manage CDPAP services. Everyone receiving health services or working as a caregiver in the CDPAP program needs to register for PPL, the statewide fiscal intermediary for the program. As an approved PPL facilitator, DHCare is helping make the transition to PPL effortless. Here is a detailed guide on how to transition to PPL with DHCare health care agency in NYC.
What is PPL?
Public Partnership, LLC (PPL) has been serving as a leading financial management service provider since 1999 throughout 20 states. Recently, the organization has been certified by the New York State Department of Health (NYSDOH) as the only fiscal intermediary (FI) for statewide CDPAP consumers. As of April 1, 2025, the organization has started to provide services according to the new governance.
Why this Transition to PPL?
The World Health Organization (WHO) has been expressing concerns over indiscipline and mismanagement in the customer-centric health services for the past several years. According to a report, in self-directed and similar services, consumers often remain unaware of the risks associated with their treatment.
In addition, they fall prey to unnecessary tests. The report also notes that about 40% of health spending by consumers gets wasted due to the inefficiency of several forms.
Reports of similar types have flooded the NYSDOH over the previous few years. To ensure better management, treatment, and fiscal facilities, the organization has reorganized the CDPAP program. Removing all the small and unreliable health care facilities from delivering CDPAP services, the organization has initiated the transition.
What to Change with this Transition to PPL?
This is what’s raising concern among consumers: how their health services will change. In two words, the answer is, not much. The transition only asks:
For Consumers
- Registering before May 15, 2025
- Visiting PPL’s predetermined resources to request a transition
- Allowing PPL to work as a co-employer to manage payments to consumers’ PAs
For Personal Assistants
- Registering before May 15, 2025
- Tracking time using PPL’s system
- Allowing PPL to manage their payrolls
What Will Remain Unchanged?
Okay, if the transition is just about registering and allowing PPL as FI, is there anything to worry about at all? No. You can carry on with how you choose a person as your personal assistant and receive services from them. Nothing will change from the list below:
- Your independence in choosing a personal assistance
- Your current services and assistants, if not found ineligible by PPL
- Your treatment hours
How to Switch to PPL?
There are several ways through which you can register for PPL. Either new or existing, every CDPAP consumer and personal assistant can opt in to PPL:
Over the Phone
PPL has provided several active phone numbers on their official websites to ease the registration process for consumers. All the numbers are kept open round-the-clock, letting patients and assistants reach guidance anytime.
Through Online
You can also register for PPL by visiting the official PPL website. Before accessing the official registration forms, visitors have to go through a sign-up process. They can sign up through their email or phone.
You can sign up with your email address from here, and with your phone from here.
Via CDPAP Facilitator
To streamline the registration process, PPL has assigned many community-based organizations throughout New York State as their CDPAP facilitators. The cardinal reason to enroll these facilitators is to provide local and culturally competent support. DHCare has made the PPL facilitator’s list as a competent and trusted health care service provider.
By Attending Registration Sessions
PPL runs scheduled virtual and in-person sessions with an aim to provide an easy PPL transition. Anyone who prefers a person-to-person support system for registration can go to and check the upcoming session schedules here.
How to Register PPL with DHCare NYC?
Approved as a PPL facilitator, DHCare ensures a streamlined PPL transition. Here is a step-by-step process:
Submit a Form Requesting PPL Transition
At first, visit the DHCare website and find the PPL submission form. Carefully fill up the form based on your current service status: non-existing or existing, desired service type: self-directed or facility provided, selected personal assistant’s details, and so on.
Receiving PPL’s Approval
After submitting your form via the DHCare website, wait until PPL responds to your request. It may take a couple of days to a week for DHcare to review your submission and assign DHcare as your CDPAP facilitator.
Await a Call from DHCare’s Support Team
As soon as PPL assigns us as your facilitator, a specially trained transition assistant from DHCare will contact you. You will be guided through the next steps of your registration.
How to Transition to PPl as a Personal Assistant (PA)?
The only thing a PA needs to become PPL registered is an association with the patient. That means a patient must claim that the PA is currently attending to them. This can be achieved in two ways:
- By a Designated Representative (DR): Directly calling a PPL or Facilitator agent
- By PPL or CDPAP Facilitator: Requires submission of three consumer-identifying documents

What if a Consumer or Caregiver is Not Yet Registered?
The New York State Department of Health (NYSDOH) primarily asked all CDPAP consumers and caregivers to register in PPL before March 28, 2025. However, after the initial rollout of the online and in-house registration, several problems came to light. One of them is about caregivers who are not fully paid by their previous FI, and consequently, can’t register for PPL.
To address and solve such issues, a new court order has been passed. It allows unregistered caregivers and consumers to keep working with their previous FI and solve intermediate issues. Giving them the window to make the shift, the court has extended the deadline for PPL till May 15, 2025.
Conclusion
While the concern over PPL transition is high, nothing about how patients receive CDPAP service changes. The transition also doesn’t put a hassle on either a caregiver or a patient, if done through a facilitating elderly health care agency in NYC, like DHCare. All you have to do is confirm us as your PPL facilitator. You will get guidance and assistance to fill up the necessary forms and meet every requirement.
